continuity of care

We believe that continuity of care is one of the most important parts of good general practice.

Seeing the same doctors over time helps build trusted relationships and allows clinicians to understand patients in the context of their lives, families, and health history.

This helps ensure care is more personalised and that changes in health can be recognised earlier.

Strong continuity of care also helps patients manage long-term health conditions more effectively and supports better health over time.

To support continuity of care, we encourage patients to have two regular GPs recorded on their file.

While it is not always possible to see the same doctor at every appointment, having two familiar doctors helps ensure your care remains consistent and that clinicians who know you and your health history are involved in your care over time.
